It's somewhat amazing how many people recognize this game despite how incredibly old it is... Today's Ancient DOS Game is Alley Cat, a quirky action game about feline antics and promiscuity, developed primarily by Bill Williams for Synapse Software with the PC version published by IBM in 1984.
As a side note, to get this game's PCjr graphics and sound working properly in DOSBox, you'll want to get the latest patch from this forum thread over at the VOGONS forums:

Additional Information and Corrections:
* Here's a link to the section on the DOS Games website you can download a patchable copy of this game from:
https://www.dosgames.com/game/alley-cat/
* I noticed during editing that, with the patch applied, it sort of converts the PCjr sound to Tandy sound, thus if you leave the Tandy sound mode set to "auto" in the config file it should still work. For some reason I didn't think this was the case while I was testing earlier. *shrugs*
* If anyone has any info on why so many legit DOS gaming websites consider this game freeware now, please let me know!
